Specialized Community Health Services
Specialized Community Health Services provide homelessness, mental health and substance use supports for people managing complex challenges, including HIV, dementia, gender transition, or justice involvement. We offer outreach, care coordination, crisis management, and life skills development, helping clients live independently. Services across Toronto, York Region, Simcoe County, and Muskoka Region.

- 16 + program-dependent
- Individuals experiencing complex mental health, physical health challenges, substance use, addictions, behavioural issues, dementia, or homelessness

Programs Available
Behavioural Supports and Specialized Geriatric Services
Behavioural Supports and Specialized Geriatric Services provide a variety of services, including behaviour supports, crisis outreach, and geriatric mental health outreach to older adults and seniors who are living with dementia, mental health, substance use or other neurological conditions.

Mental Health and Homelessness Services is a group of specialized services that provide highly accessible, flexible Intensive Case Management services from a multidisciplinary approach, including a female–identified specific drop–in and case management program.

The Toronto Transitional Age Youth (TAY) Services
The Transitional Age Youth (TAY) Services provide free case management, care navigation, peer support, mental health, and social groups for youth 16-29 years old experiencing physical, mental health challenges, substance use challenges, and homelessness.

York Region South Simcoe Intensive Case Management
Intensive case management teams provide one-to-one support to individuals experiencing mental health and/or substance use issues that impact their lives. This support is provided through service coordination, system navigation, psychosocial support, crisis prevention and intervention, personalized support plans, life skills support, and connecting people to resources that support their individual recovery and well-being.

York Region Street Outreach and Services Network
This service is for people who are unhoused, precariously housed or living in poverty in York Region. A mobile outreach van operates from 2 pm to 9 pm seven days a week.

Complex Care and Personal Support Services
LOFT's Complex Care Services offer permanent, transitional and in some cases, respite housing and support to seniors aged 55 and older, with around-the-clock specialized assisted living services across Toronto, York Region, Simcoe County, and Muskoka Region.
Supportive Housing Services
Supportive Housing Services assists individuals facing complex mental health challenges, substance use issues, and homelessness. We utilize a Housing First approach, offering case management, residential support, and life enrichment to help individuals achieve personal goals and build community.
